Effective immediately, please begin using the revised Utah Application for Disability Insurance, form DIAPP(7/07)UT. This new version contains revised medical questions as required by the Utah Department of Insurance. You may download it from The Standard’s IDI website at www.standard.com/di under Forms/Application Forms.
With the exception of applications that are currently pending in Underwriting, prior versions of the Utah application can no longer be accepted. Please discard any earlier copies you may have and begin using the new application immediately. Earlier versions will not meet the requirements of the Utah Insurance Department. |
How Utah TeleApps
will be impacted |
If you use the TeleApp process, please note that all teleinterviews conducted on and after Friday, November 9, 2007, will be printed on the new form, DIAPP(7/07)UT.
To avoid having to go back to your clients to obtain new forms and signatures, you must begin using the new form immediately. If you have any pending TeleApplications for which the previous Utah application version was used, the teleinterview will need to be completed prior to November 9. |
